Navigating The Database

PURPOSE:

A Cable Request Form must be filled out for cable installation approval and obtaining labels. It ensures that crucial information is stored in the databases.

PROCESS:

The person in charge of the specific system, i.e. Main Injector, Tevatron etc., is responsible for filling out the Cable Request Form for the cables they need pulled. This form is to be sent via email to Installation Manager (IM) Jim Ranson, George Krafczyk, and Database Manager (DBM) Evelyn Aponte.

All questions are welcome. Hand-filled forms are accepted, but we do prefer electronic transmission. If you are turning in a hard copy go to Contact Personnel below for drop off locations.

HOW TO FILL OUT YOUR FORM:

  1. Select the appropriate "Cable Request Form" below.
  2. Select the Excel workbook "Reference Files" below. It contains the codes and examples necessary for completing the Request Form.
  3. Complete the form while simultaneously viewing the reference files.
  4. A pre-addressed email link is provided below for submitting your completed request form.

    Notes:

    1. The Function, Origin and Destination Codes are made up by the requester using any 6 characters which make sense to him/her. Origin and Destination examples are supplied in Table 2.
    2. For the remaining columns, codes are supplied in Tables 1, 3, 4, and 5.
    3. If a needed code does not exist, type or write out a description in the respective cell (or add a note, if hand written). The DBM will add and assign new codes as well as update the Tables as necessary.
