Surface concrete had spalled and warning cones placed in the area had been moved to facilitate movement of equipment but had not been replaced. An employee received a finger puncture wound and burn when the welding electrode he was using slipped from his grasp and he reflexively caught it, causing the electrode tip to penetrate his glove and pierce the skin of his finger. An employee suffered a significant shoulder injury from slip on ice in parking lot The employee was walking in a parking lot and slipped on a patch of ice which he did not see. The employee flailed his arms to regain his balance and had onset of right shoulder pain. He did not actually fall to the ground. Later determined that the employee had suffered a torn rotator cuff from the event. An employee received 1 st and 2 nd degree burns to right hand when he touched a 3-phase motor which was overheated. The elevated temperature of the motor housing was caused by incorrect wiring (mis-phased) in an extension cord being used to power the motor. An employee developed an infected wound a week after cutting his leg on the metal frame of a computer room tile floor while laying down on the floor to plug in equipment. The employee initially thought the wound was too superficial to require reporting, but it failed to heal and became red, prompting the employee to report to the Medical Office.
